Institutional racism, also known as systemic racism, is the result of dynamics from institutions – not directly, drawbacks and privileges concerning base as a topic. The integrality is part of data in collective health studies and shows rising plateau regarding quality and admission. Even though all pregnants have the right of decent (quality and quantity) appointments during the pregnancy, labor and puerperium and there were improvements on prenatal attention, different sources showed that black women still being the most harmed considering access to the health service. The aim of this study was to analyze the dimension of social markers, as race, color, social class and scholarity level can have an impact on pregnant’s health care during prenatal, in (previous) survey made in two districts from Alvorada, located on the metropolitan region of Porto Alegre/RS from reports of women. The data production instruments were a sociodemographic identification questionnaire, interviews with a semi-structured script and notes in a field notebook. Two black women depoiments were selected to compound this article. Data were treated using thematic analysis with the construction of theoretical and empirical categories. The results showed that black pregnant women have told to suffer obstetric violence and racism and fear of not being able to have a companion during childbirth due to the COVID-19 pandemic. The reports obtained have an important role and reveal an even darker face of institutional racism: the devel-opment of fear and vulnerability, even in public and private health services, warning that is important to teach about health service to build a culture that does not tolerate any kind of prejudice.
