    MUDANDO CONTINUAMENTE E AGREGANDO PARA CRESCER


    Em tempos de mudança enfrentamos uma pletora de artigos enviados e a possibilidade cada vez maior de termos a divulgação de nossa produção.
    Acrescentamos novas sessões onde estão contemplados resumos de teses bem como resumos de artigos publicados em revistas internacionais de autores locais. Reconhecendo a necessidade de publicação em revistas internacionais por parte de nossos pesquisadores promovemos veiculação destes artigos para nosso conhecimento.
    A resposta do Serviço de Endocrinologia demonstra a imensa possibilidade que nossos Serviços tem de poder divulgar suas pesquisas e ao mesmo tempo promover educação continuada. 
    Estes dois números fazem parte da iniciativa anterior de ter assuntos de fundo para revista. Sem dúvida estamos fazendo com um dos grupos mais produtivos e representativos da nossa pesquisa sendo um exemplo para todos que querem crescer cientificamente.
    Continuamos com a publicação de artigos gerais, além dos resumos de teses e dos artigos publicados no exterior, fazendo com que cada professor sinta-se responsável pela qualidade de nossa revista.
    Portanto, temos o dever de encaminhar os trabalhos para que possamos desfrutar deste conhecimento gerado e estarmos orgulhosos de nossa publicação internacional e das teses orientadas.

    E d i t o r i a l


    Este número da revista do Hospital de Clínicas de Porto Alegre é de responsabilidade do Serviço de
    Endocrinologia. Procurou-se apresentar artigos que refletissem as atividades desenvolvidas ao longo dos
    últimos 25 anos. Desde o início, os professores do Serviço têm implementado protocolos de assistência
    voltados para os problemas considerados os mais importantes pela sua elevada prevalência e relevância
    clínica. Assim, foram criadas rotinas para o atendimento da cetoacidose diabética e iniciou-se também
    uma avaliação sistemática das complicações crônicas do diabete. Isto motivou a criação de linhas de
    pesquisa específicas que foram desenvolvidas persistentemente ao longo destes anos e culminaram na
    publicação de vários trabalhos originais em revistas internacionais. Neste número, um trabalho desenvolvido por alunos de iniciação científica descreve a prevalência atual dessas complicações, e observa-se uma
    redução significativa em relação aos estudos iniciais, traduzindo as melhores condições de diagnóstico e
    tratamento. É apresentada também uma análise da relação entre a estrutura glomerular e a função renal
    em pacientes diabéticos, na tentativa de identificar os marcadores precoces do comprometimento renal no
    diabete.
    Os novos conceitos de diagnóstico do diabete melito e das alterações da homeostase glicêmica, assim
    como as novas modalidades de tratamento da hiperglicemia e das co-morbidades freqüentemente associadas nos pacientes com diabete melito tipo 2 – hipertensão arterial, obesidade e dislipidemia – são também
    apresentados em artigos de revisão.
    As doenças da tireóide representam as endocrinopatias mais freqüentes e portanto, também foram
    contempladas. O nosso grupo vem se dedicando a responder perguntas encontradas freqüentemente na
    prática clínica e ainda não respondidas satisfatoriamente. O hipertireoidismo por doença de Graves é uma
    das doenças de tireóide mais importantes, tanto pela sua freqüência elevada, como pela morbi-mortalidade associada. O tratamento usualmente praticado baseia-se na administração de medicamentos antitireoideanos por um tempo prolongado de 1 a 2 anos. Idealizou-se um tratamento simplificado que consiste
    na administração de uma dose única de iodo radioativo. Esta proposta é apresentada neste número, tendo
    sido analisada em um estudo clínico randomizado controlado que resultou em publicações em revistas
    conceituadas e obteve grande aceitação na literatura internacional. A introdução de técnicas de Biologia
    Molecular em nosso laboratório a partir de 1996 permitiu que fossem estudados aspectos relacionados à
    possível transformação maligna dos bócios assim como a detecção precoce do carcinoma medular de
    tireóide hereditário.
    A partir de 1988, o Serviço de Endocrinologia do Hospital de Clínicas de Porto Alegre passou a ser o
    centro de referência para a distribuição de hormônio do crescimento às crianças portadoras de deficiência
    deste hormônio. Estão incluídos artigos que analisam os procedimentos diagnósticos envolvidos na avaliação da baixa estatura, como também a experiência do tratamento e acompanhamento da coorte de
    crianças com deficiência de hormônio de crescimento tratados em nosso Hospital.
    O emprego amplo dos métodos de imagens favoreceu a detecção de alterações morfológicas em
    glândulas endócrinas, denominadas genericamente de incidentalomas. Estes incidentalomas têm sido
    relatados freqüentemente na glândula adrenal e a sua avaliação precisa, importante para definir a relevância clínica deste achado, é descrita neste número.
    Por fim, são apresentados alguns resultados da Unidade de Ginecologia Endócrina, criada em 1989,
    e que vem se dedicando ao estudo dos problemas endocrinológicos relacionados à mulher, especialmente
    os aspectos patogênicos, a avaliação e o tratamento dos estados hiperandrogênicos e do climatério. Alternativas práticas para o tratamento do climatério e alterações metabólicas encontradas nesta etapa são
    comentadas em dois artigos.
    Os trabalhos apresentados neste número da Revista do HCPA representam muitos anos de dedicação dos professores do Serviço de Endocrinologia aos princípios acadêmicos da assistência e pesquisa em
    harmonia com as atividades de ensino de graduação, especialização e pós-graduação. Os alunos de graduação têm sido estimulados a se engajarem ativamente nestes processos. Muitos destes alunos ingressaram
    em programas de residência e posteriormente seguiram formação acadêmica formal e hoje fazem parte do
    corpo docente de faculdades de medicina no País e no exterior.
    As atividades de pesquisa aqui representadas dependeram fundamentalmente do empenho e da
    criatividade dos alunos de pós-graduação que transformaram projetos em resultados originais e inovadores.
    O Programa de Pós-graduação em Ciências Médicas: Endocrinologia, Nutrição e Metabolismo iniciou as
    suas atividades em 1996 e até o momento já titulou 22 mestres e 14 doutores de forma crescente e
    progressiva, conforme apresentado na Figura 1. Todos os egressos do doutorado estão exercendo funções
    em instituições acadêmicas.

    Todas as dissertações e teses originaram publicações em revistas internacionais relevantes de elevado índice de impacto. Como pode se perceber nas Figuras 2 e 3, o número e a qualidade das publicações vêm
    aumentando progressivamente e apresenta uma boa correlação com o número de dissertações e teses apresentadas (R=0,833, P=0,04; R=0,911, P=0,011, respectivamente).

    Ao longo destes anos de atividades o Serviço de Endocrinologia tem recebido apoio contínuo da Administração do Hospital de Clínicas de Porto Alegre que junto com recursos obtidos de agências de fomento à pesquisa – especialmente Fundação Coordenação de Aperfeiçoamento e Ensino - propiciaram as condições de infra-estrutura e área física ideais para a realização das atividades de ensino e pesquisa existentes.

    A PUBLICATION FOR RESEARCH AND EDUCATION

    Revista HCPA is the journal of the Medical School at Universidade Federal do Rio Grande do Sul. For this reason, perhaps it deserves special treatment by the professors of this School in our University.

    The difficulties and the possibility of consolidating a continuing publication, and most importantly, apublication that has contributions of attested quality, are being currently discussed in the scientific literature worldwide. We have an exceptionally experienced body of teachers, physicians and other professionals working at Hospital de Clínicas de Porto Alegre. Thus, our research activities and topics of continuing medical education have guaranteed space in this journal. On the other hand, we are aware of the need to have papers published in journals of greater impact, which ensure better scores for graduate programs. However, the local public should learn about our research activities and our internationally published papers. Thus, we are introducing an innovation to Revista HCPA with the inclusion of the following sections: Abstracts of thesis and dissertations presented at Graduate Programs at UFRGS and Abstracts of papers published internationally by local authors.

    It is only by bringing together and allowing as many authors as possible to present their research that we will spread knowledge and improve education.

    Revista HCPA, the journal of the Medical School at Universidade Federal do Rio Grande do Sul, has been published for more than 20 years uninterruptedly. If the possibilities provided by the improvement of our journal are understood, we will certainly receive contributions of increasingly more authors. Thus, we invite all those who are interested in collaborating with our Revista HCPA to help in the consolidation of this journal.

    Revista HCPA: This is our journal

    In the years that I have served as Editor at Revista HCPA I have witnessed the great effort made by a considerable group of people to ensure the viability of our journal. Maintaining a journal like Revista HCPA is not an easy task, and this is not only because of financial reasons; there is also much ambiguity on the part of our investigators in terms of submitting their papers to Revista HCPA. Although this is understandable, taking into account the current policies of funding agencies, which have adopted a series of requirements concerning the journals in which research results should be published, on the other hand this situation leads to a paradoxical vicious circle: because it receives few submissions, the quality of Revista HCPA tends to decrease; and because of the decreasing quality, Revista HCPA receives fewer and fewer submissions. It is not uncommon to hear colleagues say that “this article is too good to be published in Revista HCPA.”

    I believe it is time for us to face this reality and define the true vocation of our Journal. Should we try to emulate international journals? Should  we focus on a more regional public? Should we favor a teaching profile, with the publication of more articles geared towards practitioners?

    Regardless of the answer, I believe that the debate concerning our Journal has to take into account an important point: Revista HCPA is our Journal. It is in our hands to make it become a powerful vehicle – for that, all we have to do is to seriously engage in submitting some of our research results, take part in Editorial Board meetings, meet the deadlines for reviewing articles, etc. If we fulfill simple goals, and if we dedicate some of our best papers to Revista HCPA, we could, in a relatively short period of time, increase its quality to a level acceptable also to funding agencies, in addition to making it become a significant forum of scientific debate.

    Thus, until the end of 2002, I invite all colleagues to engage in this debate and to reflect on the possibility of having a Journal that truly represents the scientific production of Hospital de Clínicas de Porto Alegre, a journal that circulates true knowledge and continued education and meets the demands of funding agencies. This is the time for us to look at the future of Revista HCPA and develop the strategic plan that will ensure to Revista HCPA the longevity it deserves.

    Research, assistance, and education


    In this issue of the Revista HCPA we address the subject of nephrology
    with the support of Dr. Roberto Manfro, who partakes in this journal as an
    Associate Editor. The studies published here are a small example of the
    comprehensive work that is being carried out by professionals of the Nephrology
    Services at the Hospital de Clínicas de Porto Alegre. Created in 1975, the
    Nephrology Services has maintained an up-to-date and efficient service by
    combining research - with inpatient and outpatient settings - and programs
    aimed at assistance and education.
    As a contribution to the maintenance of the quality of the Services, a
    Graduate course program in Nephrology was created also in the 1970s. The
    program operates, until today, as part of the Graduate studies in Nephrology
    of the Universidade Federal do Rio Grande do Sul. Recently, the Graduate
    course has also started a doctorate program and it also accepts students

    from schools other than the Medical, such as those from the areas of biological
    sciences.
    We would like to thank all contributions sent to the Revista HCPA
    and also to call on all authors to participate in the disseminating and promoting
    of research at the Hospital de Clínicas through this journal. The Revista HCPA
    is now also a publishing instrument at the services of the Fundação Médica
    do Rio Grande do Sul (State of Rio Grande do Sul Medical Foundation) and
    will be able to count on the support of the Foundation to extend its reach.

    Representing our qualified service

    Number 2 of the 20th volume of Revista HCPA delivers a sample of the
    activities carried out at the Gynecology and Obstetrics Service of our Hospital.
    Doctor Fernando Freitas, chief of Gynecology and Obstetrics and associate
    editor of this issue, has put together and organized a number of significant
    studies reflecting the qualified staff of doctors and researchers of the Service.
    These studies include original contributions, special contributions, and a case
    report that are altogether fitting to the betterment of discussions regarding the
    wide spectrum of matters related to Gynecology. As an example of the quality
    of the Obstetrics and Gynecology Service at Hospital de Clínicas, the maternity
    at that service is a reference center for high risk care. Patients at the maternity
    will receive care from multiprofessional teams specialized on high risk
    situations involving the mother, the fetus, and the newborn.
    The present issue of Revista HCPA brings original and special articles
    Espelhando qualidade

    as well as a case report that will certainly contribute to enhance the discussion
    concerning the wide range of topics encompassed by the field of obstetrics
    and gynecology.
    The debate should not end right here, within the pages of our Revista
    HCPA. In reaffirming its task as a medium for the publication of research
    studies, Revista HCPA operates as a means to encourage scientific studies
    carried out within the Hospital de Clínicas de Porto Alegre and outside hospital
    walls. Publishing the scientific production of our researchers allows us to
    present the qualified work that is carried out in this institution, and reflects our
    day-to-day search for the betterment and improvement of the services offered
    to our patients.

    Revista HCPA: 20 years

    Revista HCPA reaches the new millenium with a young soul: at year
    2000, we celebrate its 20th anniversary. During this time, it has fulfilled its role
    as a vehicle for the dissemination of biomedical research and health education.
    More than that, it has fulfilled its mission of giving visibility to the research
    carried out at Hospital de Clínicas de Porto Alegre, and as a result to the high
    quality of this Institution. This responsibility is renewed with every new issue of
    Revista HCPA. Our journal becomes, over and over again, a forum for scientific
    debate, gathering all of those who believe that research is intimately associated
    with quality of life.
    The publication of Revista HCPA abstracts in the Internet has prompted
    several requests for articles and subscriptions from Brazilian and foreign

    universities. Starting in 2000, the Medical Foundation at HCPA will take over
    the administration and distribution of Revista HCPA; this will certainly mean
    more subscriptions and sponsorship opportunities. Also, it will mean more
    exposure, and therefore authors should feel more encouraged than ever to
    publish their work in Revista HCPA. As we enter the last year in this millenium,
    we reaffirm our responsibilities: to make things happen and to make them
    visible; and to confirm the importance of Revista HCPA as a major link between
    peers and between our Hospital and the communities it serves.
    Finally, as we celebrate the 20th anniversary of Revista HCPA, we would
    also like to acknowledge the work of all previous editors – Professor Nilo Galvão,
    Professor Sérgio Menna Barreto, and Professor Luiz Lavinsky – the joint effort
    of all who have dedicated their time to this project has made it a lasting success.

    Good news for a new year


    As we move along with our aim of recovering and stimulating the scientific
    production of the outstanding Services at Hospital de Clínicas de Porto Alegre,
    we present the last 1999 issue of Revista HCPA. This is a collection of works
    by a highly qualified group of colleagues from the Intensive Care Center,
    coordinated by Professor Silvia Regina Rios Vieira, who has recently returned
    from a period of studies in France.
    Professor Rio Vieira has long contributed to the organization of Revista
    HCPA. Therefore, it is an honor to have her as Associate Editor in the present
    issue, which closes one year and already looks into the next. And the next
    year is full of great expectations.
    In 2000, we celebrate the 20th anniversary of Revista HCPA. Along these
    years, our journal has matured as a medium of expression for the work
    developed at our Institution. Revista HCPA continues on the Internet, and
    starting with the next issue, it will received administrative support from the
    Medical Foundation. This will result in more flexibility for obtaining other types

    of sponsorships and for selling subscriptions, opening up the scope of the
    journal.
    With such an excellent perspective, we end this volume with yet another
    invitation to all colleagues to submit their work to Revista HCPA. We are open
    to all individual researchers, and to those who would like to propose issues
    that are focused on the work of specific groups.

        New section to Discussion

         The concept of a University Hospital implies the ideas of assistance,
    teaching and research. However, it is clear that the concept behind Hospital
    de Clínicas de Porto Alegre is much wider. At our Hospital, we are also
    concerned with community programs, with sharing pertinent information, and
    with developing projects that are important for the communities that we serve.
         Despite the tight resources, we work hard to complete the projects we start.
    In our experience with Revista HCPA, we have noticed that HCPA staff
    are ready to respond promptly to the challenges they are presented with.
         This is reflected in the enthusiasm with which our colleagues have accepted
    the task of Associate Editor, helping to organize each issue of our journal
    around a specific topic. The participation of these Associate Editors has meant
    more consistency and higher scientific content, with more original,
    experimental, work. Also, it has given more visibility to the research developed
    in our Institution, and has ensured space for relevant clinical reviews.
          Therefore, Revista HCPA has recently been receiving a larger number of
    inquiries and requests for regular subscriptions.

    A great satisfaction 

    Our journal has risen a greater deal of interest than what we have foreseen in the short time since its reformulation. The idea of an associate editor, chosen among Scientific Council Members, was very  important for increasing the scientific and educational level of the journal. This is a way of publicizing our production as qualified research groups, and to acknowledge our contribution to continued medical education. We reach colleagues who are interested in state-of-the-art subjects and also those who look for review and in-depth information regarding a specific subject.

    There have been many favorable reactions to the participation of an associate editor of thematic issues. Initially, the associate editors are invited among Scientific Council members, so that all members are involved in the process of change. We have tried to select subjects or specialties which have a broad impact; also, we have looked for editors who have had publications of impact within the last years, in national and international
    journals. In 1998, our associate editors were Professor Sérgio Menna Barreto (pneumology), Professor Rogério Friedman (endocrinology), and presently, Professor Themis R. da Silveira (hepatology). The next issue will offer articles in psychiatry, coordinated by Professor Paulo Abreu.

    Finally, it is with great satisfaction that we have received the support and the explicit approval of professors who are interested in contributing
    with our journal and in participating as associate editors. Therefore, it seems that we are indeed raising the interest of those who are enthusiastic about the development of ideas and knowledge. We thank all those who have manifested their support, and we renew our invitation, so that they keep contributing their work to Revista HCPA.

    A TIME FOR REFLECTION

    The year of 1998 is especially meaningful for Revista HCPA, because it is the year in which we celebrate the 100th anniversary of our School of Medicine. In the following pages, our readers will find enough reason to
    reflect about the significance of such a date. In his editorial, Professor Pedro Gus briefly outlines the history of the School of Medicine, and at the same 134 Revista HCPA 1998;18 (2) time emphasizes the persistence and will of students and professors, which enabled the school to overcome hard times. In addition, the editorial written by Professor Sérgio Pinto Machado, Chief Executive Officer of Hospital de Clínicas de Porto Alegre, stresses the importance of the relationship between the School of Medicine and the Hospital. Taken together, the ideas foregrounded in these editorials are extremely relevant for Revista HCPA. In a number of ways, Revista materializes the relationship between these two worthy Institutions. This is a publication founded upon the same will, and impelled by the same effort, described by Dr. Gus.

    The contents of this issue of Revista HCPA, in which Dr. Rogério Friedman is an invited Editor, reflect the high-quality academic work carried out at HCPA. Dr. Friedman highlights innovative aspects of the research being developed right here in the field of Endocrinology, which, as can be seen from the set of papers he selected, has an impressive repercussion world-wide. We hope that these works may serve as incentive for our collaborators, so that, in the coming year, Revista HCPA may grow with the submission of many original articles which will mirror the excellence of the scientific work developed both at the School of Medicine and at HCPA.
