The protein/creatinine ratio in hypertensive pregnant women

OBJECTIVE: To determine optimal protein/creatinine ratio for positive predictive value of significant proteinuria in pregnant women with systemic arterial hypertension.
PATIENTS AND METHODS: We carried out a review of the literature with the objective of obtaining correlation coefficients of protein/creatinine ratio and 24-hour proteinuria.
RESULTS: The correlation coefficient for protein/creatinine ratio and 24-hour proteinuria was 0.94 (0.53-0.98). There was no significant difference between the results obtained at different times of the day. We present an ROC curve of the analysis of sensitivity and specificity of the ratio for the diagnosis of significant proteinuria (> 300 mg in 24 hours). Specificity and positive predictive values were of 100 % for a ratio of > 0.8. The optimum values of sensitivity (0.96), specificity (0.96), positive predictive value (0.96), and negative predictive value (0.96) for the diagnosis of proteinuria > 300 mg in 24 hours occurred when the protein/creatinine ratio was 0.5 mg/mg.
CONCLUSIONS: The protein/creatinine ratio of random urine samples from hypertensive pregnant women presented good sensitivity and specificity for the diagnosis of proteinuria > 300 mg in 24 hours and strong correlation with 24-hour proteinuria. A ratio of 0.5 mg/mg presented a significant positive predictive value for proteinuria and can be used in the diagnosis of hypertensive pregnant women.
