Homem de 27 anos, previamente hígido, veio ao Hospital de Clínicas de Porto Alegre com dor torácica iniciada há 7 dias. A dor se caracterizava por pressão no dorso, irradiada para região anterior do tórax, sem associação com outros sintomas, fatores de alívio ou agravamento. Ausência de febre, tosse ou outras queixas. Ao exame físico, não foram detectadas alterações. Eletrocardiograma (figura 1) apresentou inversão de onda T em D2, D3 e AVF. Foi realizada dosagem de marcador de necrose miocárdica – troponina I – que demonstrou valor inicial de 5.7ng/mL (referência para lesão miocárdica >0,04ng/mL), com redução para 1.7ng/mL, após 24 horas de internação. Foi realizado ecocardiograma que demonstrou coração normal.
