Introduction: The implementation of health residency programs is essential for the training of professionals aligned with the challenges of the Brazilian Unified Health System (SUS). Objective: To present and comparatively analyze the management and organization of health services within the context of two Professional Health Residency Programs (PRAPS) – one in the North region (Manaus, Amazonas) and the other in the South region (Porto Alegre, Rio Grande do Sul), highlighting challenges and potentialities. The comparison of experiences aims to contribute to the debate on improving training and optimizing health services nationwide. Experience report: This report details the authors' experiences in analyzing the challenges and critical elements of the implementation and management of residency programs, based on their roles as preceptors in a uniprofessional Intensive Care Nursing program in the North region and in a multiprofessional Collective Mental Health program in the South region. The analysis considers the pedagogical models, selection processes, and funding of both programs, revealing the influence of regional factors, the centrality of the preceptor's role, and the obstacles faced, in addition to emphasizing the relevance of effective management and institutional support for the development of these programs. Conclusion: The relevance of the PRAPS for training aligned with the principles of the SUS is reinforced. The North-South comparative analysis demonstrates the need for regionalized approaches in management and implementation, considering cultural and contextual specificities. Effective management, preceptor support, and local adaptation are crucial for qualifying the SUS workforce and improving health care, offering subsidies for permanent education and professional qualification strategies in health in the country.
