O trato iliotibial corresponde a uma bainha fibrosa longitudinal localizada ao longo da região lateral da coxa e serve como uma importante estrutura envolvida no movimento dos membros inferiores. A Síndrome do trato iliotibial proximal é pouco relatada na literatura médica, mas é uma condição que ocorre mais frequentemente em corredoras, obesas e idosas, podendo levar a limitação da movimentação da paciente. Estudos biomecânicos apontam que uma maior relação entre largura da pelve e o comprimento do fêmur, que ocorre em mulheres, provoca uma maior adução do quadril, gerando uma sobrecarga na musculatura lateral a fim de manter o equilíbrio da pelve. Relatamos o caso de uma mulher de 46 anos com dor no quadril direito há 2 anos, com piora ao andar e ao subir escadas cuja ultrassonografia demonstra a lesão da inserção proximal do trato iliotibial, diagnosticando a síndrome do trato iliotibial proximal.
