Disfunção Temporomandibular associada à Síndrome de Ehlers-Danlos

Objetivo: Relatar um caso de Disfunção Temporomandibular (DTM) associada à Síndrome de Ehlers-Danlos (SED), subtipo hipermóvel. Relato do caso: Paciente do sexo feminino, branca, 30 anos, com queixa de dor crônica intensa na região da articulação temporomandibular (ATM), nos músculos mastigatórios e no complexo crânio-cervical, além de ruídos articulares e episódios diários de luxações e subluxações, especialmente ao bocejar. Na história médica, relatava diagnóstico prévio de SED hipermóvel, com episódios recorrentes de quedas, subluxações e luxações em múltiplas articulações. O diagnóstico de DTM foi estabelecido com base no protocolo DC/TMD e em exames de tomografia computadorizada e ressonância magnética da ATM, que revelaram alterações morfológicas e funcionais articulares. A abordagem terapêutica adotada foi interdisciplinar, incluindo uso de placa estabilizadora oclusal, fisioterapia especializada, acompanhamento psicológico, controle farmacológico e orientações para manejo de hábitos parafuncionais. Discussão: A DTM pode estar associada à hipermobilidade articular, como observado na SED, o que impõe desafios adicionais ao diagnóstico e manejo. São escassos os estudos que abordam a condução clínica de pacientes com essa associação. Conclusão: A paciente relatou melhora significativa na qualidade do sono, na dor orofacial e crânio-cervical, bem como maior estabilidade funcional da ATM, embora persista a flutuação dos sintomas. Este caso reforça a importância de uma abordagem ampliada e interdisciplinar, considerando a DTM como possível manifestação de condições sistêmicas, como a SED.
