Assessment of methodological quality of guidelines for early childhood caries prevention

Abstract

Aim: To assess the methodological quality of clinical practice guidelines (CPGs) focused on early childhood caries prevention. Materials and methods: A systematic literature search was conducted in databases and on guideline organizations' websites. Two reviewers selected evidence-based CPGs containing recommendations for caries prevention in deciduous teeth of children under 6 years old, without restrictions on country, language, or publication date. Three trained assessors applied the AGREE II instrument to each selected guideline, evaluating the methodological quality and providing recommendations for use. Results: Six guidelines were assessed, showing variations in methodological quality. None of the CPGs reported the use of a reporting guide. Overall, the CPGs received higher scores for the domains of "clarity of presentation" (71.0%, 95% CI 56.1 - 85.9), and "scope and purpose" (68.2%, 95% CI 50.9 - 85.4) and "stakeholder involvement" (62.3%, 95% CI 41.8 - 82.9); and lower scores for the domains of "rigor of development" (59.2%, 95% CI 39.4 - 78.9), "applicability" (58.7%, 95% CI 32.8 - 84.6) and "editorial independence" (57.2%, 95% CI 31.4 - 83.0). One guideline received a recommendation for use, four received recommendations with modifications, and one was not recommended. Discussion: The evaluation of CPGs for early childhood caries prevention revealed variability in methodological quality. Conclusion: It is necessary to adhere to reporting guidelines, improve the reporting of some domains, and enhance the presentation, especially in the applicability and editorial independence of CPGs focused on early childhood caries prevention.
