Analysis of the correlation between headache and signs and symptoms of temporomandibular disorder in university students

Abstract

Aim: To analyze the correlation between the impact of headache and the intensity of signs and symptoms of Temporomandibular Disorder (TMD) in university students. Materials and Methods: Cross-sectional study with a sample of 86 Dentistry students, over 18 years old. We used the Headache Impact Test, the Protocol for Determining Signs and Symptoms for Multidisciplinary Centers and the Fonseca Questionnaire.  To analyze the data, we used the non-parametric Fisher's Exact test, Pearson's correlation coefficient, Spearman's rank or order correlation coefficient and simple linear regression analysis. Significance was 5%. Results: Most participants presented, upon awakening, pain in the facial muscles, pain in the neck and sensitivity in the teeth. The impact of headache was correlated with joint pain, neck pain and ear pain. The degree of the most present dysfunction was mild. Discussion: The signs and symptoms found in this study were mostly mild or moderate, which may be related to the young sample, as well as the characteristic of pain related to chronic muscular TMD. The most common primary headache subtype related to TMD is migraine, followed by tension-type headache. Conclusion: There are clinical overlaps between some painful disorders and headache conditions that can make the diagnosis and treatment process more complex. Important pain mechanisms that contribute to the close association and complex relationship between TMD and headache as processes of peripheral sensitization need to be further studied.
